Elevated homocysteine levels have been associated with an increased risk of ischemic stroke in various populations. This study aims to find the association between homocysteine and ischemic stroke in the Pakistani population. The study recruited 100 cases with ischemic stroke and 100 controls without any history of stroke from two tertiary care hospitals in Pakistan. Homocysteine levels were measured in both groups using a chemiluminescent microparticle immunoassay. The mean homocysteine level was significantly higher in cases compared to controls (24.64±7.41 µmol/L vs. 12.02±3.59 µmol/L, p<0.001). Logistic regression analysis revealed that high homocysteine levels were independently associated with an increased risk of ischemic stroke (OR=3.62, 95% CI: 2.02-6.50, p<0.001) after adjusting for potential confounders. This study provides evidence for the association between homocysteine levels and ischemic stroke in the Pakistani population. High homocysteine levels were found to be an independent risk factor for ischemic stroke.
