Frequency of Intraoperative Bile Duct Injury During Laparoscopic Cholecystectomy

Bile duct injury is a serious complication of laparoscopic cholecystectomy that may result in substantial morbidity, repeated interventions, and prolonged hospitalization. Early intraoperative recognition is essential for timely management and improved outcomes. Objective: To determine the frequency of intraoperative bile duct injury during laparoscopic cholecystectomy in patients presenting at Lady Reading Hospital, Peshawar. Methods: This study was conducted on 219 patients aged 18 to 65 years, of either gender, diagnosed with cholelithiasis and undergoing laparoscopic cholecystectomy from 05-July-2024 to 05-January-2025 in the Surgery Department of Lady Reading Hospital-MTI, Peshawar. Patients with bleeding disorders, obstructive jaundice, gallbladder carcinoma and pregnancy were excluded. Intraoperative bile duct injury was assessed by visual inspection for bile leakage or inadvertent duct transection. Data were analyzed using SPSS 20. Post-stratification Chi-square or Fisher's exact test were used at a 5% significance level. Results: The mean age of the 219 patients in the present study was 46.22±12.74 years, and the mean BMI was 25.79±3.21 kg/m2. There were 117 (53.4%) male and 102 (46.6%) female patients. The frequency of intraoperative bile duct injury was 6 (2.7%) patients. No statistically significant associations of intraoperative bile duct injury were observed with demographics and comorbidities. Conclusion: The frequency of intraoperative bile duct injury during laparoscopic cholecystectomy in this study was 2.7%.
