Assessment of Night Shift Work on Depression and Sleep Disturbance among Nurses in Tertiary Care Hospital Lahore Pakistan

Night-shift work may disturb circadian rhythm, impair sleep quality, and increase psychological stress among nurses. These effects may be more pronounced in nurses with frequent or permanent night-shift duties. Objective: To assess depression and sleep disturbance among nurses working in a tertiary care hospital in Lahore, Pakistan, and to determine their relationship with night-shift frequency. Methods: This descriptive cross-sectional study was conducted at Jinnah Hospital, Lahore, Pakistan, from September 2025 to February 2026. A total of 200 registered nurses working in medical, surgical, pulmonology, gynecology wards, and the Emergency Department were selected through simple random sampling. Data were collected using a structured self-administered questionnaire. Depression was assessed using the Beck Depression Inventory, while sleep disturbance was measured using the Sleep Quality Scale. Data were analyzed using IBM SPSS version 26.0. Categorical variables were expressed as frequencies and percentages, while continuous variables were presented as mean ± standard deviation. Independent-samples t-test was applied to compare day-shift and permanent night-shift nurses, and Pearson correlation was used to assess the relationship of night-shift frequency with depression and sleep disturbance. A p-value <0.05 was considered statistically significant. Results: Among 200 nurses, 145 (72.5%) were female. Overall, 82 (41.0%) nurses were working day shifts, 68 (34.0%) rotational shifts, and 50 (25.0%) permanent night shifts. The mean Beck Depression Inventory score was 21.84 ± 9.62, while the mean Sleep Quality Scale score was 52.71 ± 14.88. Permanent night-shift nurses had significantly higher depression scores than day-shift nurses (27.48 ± 9.10 vs. 17.36 ± 7.44; p<0.001). Sleep disturbance scores were also significantly higher among permanent night-shift nurses compared with day-shift nurses (63.54 ± 12.45 vs. 45.12 ± 11.32; p<0.001). Night-shift frequency showed a positive correlation with depression (r=0.52; p<0.01) and sleep disturbance (r=0.61; p<0.01). Depression and sleep disturbance were also strongly correlated (r=0.68; p<0.01). Conclusion: Permanent night-shift work was significantly associated with higher depressive symptoms and greater sleep disturbance among nurses. Increasing night-shift frequency was linked with worsening depression and poorer sleep quality, supporting the need for structured shift scheduling, adequate rest periods, sleep-health education, and regular psychological screening among nurses.
